* Trainee GPs and young GPs are considered as junior doctors until 5 years after graduation.
Please be advised of the following.
The deadline for on-line registration is: the 5th September 2011.
**Registrations received after September 5th, 2011 will be processed on-site.
Registration fee for WONCA direct members, delegates, junior doctors,
trainees and students includes:
Admission to all scientific sessions,
Admission to poster exhibition and technical exhibition,
Opening ceremony and welcome reception,
Conference materials (delegates bag, final programme, abstract book, etc.),
Coffee breaks and lunches (Friday September 9th and Saturday September 10th, 2011) in the conference area.
Registration fee for accompanying persons includes:
Opening ceremony and welcome reception
Coffee breaks and lunches (Friday, September the 9th and Saturday, September the 10th, 2011) in the conference area.
The specified documents are to be submitted jointly with the registration form:
graduation diploma (junior doctors)
letter of confirmation from the head of the department and a copy of the student ID (the students)
member certificate (Wonca direct members)

Cancellation policy for registration and refunds
Cancellation must be notified in writing to the secretariat by fax (+48 12 431 21 44) e-mailed to
registrationwonca2011@ziz.com.pl, under the following conditions:
For cancellations made up to the 31st of , 50% of the conference fee will not be reimbursed/will remain due.
For cancellations made after the 31st of July 2011, the entire conference fee will not be reimbursed/will remain due.
